Consider a June 2006 case reported by the New York Times where three workers were injured when an eight-car subway train they were traveling on slammed into a protective block in a Canarsie train yard. The subway operator apparently suffered a seizure while trying to park the train after it had been washed down at the yard. Luckily, there weren’t other passengers on the train at the time, but the three subway workers involved suffered injuries. The Metropolitan Transit Authority issued a report in 2007, stating that 238 NYC subway workers had been killed in work accidents since 1946. The majority of cases occurred when the victim was hit by a train while working on the tracks, but still others were electrocuted on the third rail or died from falls or during train collisions. Half of the total number of deaths that they reported occurred in the 1940s and '50s alone, indicating that subway safety has steadily improved throughout the years.

In August of 1991 a drunk train operator, Robert Ray was going more than 40 mph in a 10 mph area and caused 200 people to be injured and 5 people to die when his No. 4 train derailed near Union Square. It was the worst subway accident in 63 years, disrupting service for 6 days while workers scrambled to clear the wreckage. Robert Ray was later convicted of manslaughter and sentenced to 15 years in prison.
