In August of 1991 a drunk train operator, Robert Ray was going more than 40 mph in a 10 mph area and caused 200 people to be injured and 5 people to die when his No. 4 train derailed near Union Square. It was the worst subway accident in 63 years, disrupting service for 6 days while workers scrambled to clear the wreckage. Robert Ray was later convicted of manslaughter and sentenced to 15 years in prison.
